Registration

The necessary paperwork for enrollment is provided below. We ask that all forms be completed, signed, and mailed to our business office. Please keep a copy for your records. The registration and risk and release forms are required in order for us to register your camper and allow us to legally provide care. The health form is needed so that we may create a safe environment for your child and allow for medical services in the event of an emergency. The Faith, Hope, and Charity waiver is required because we are using gym space at this school as a base for our camp. This form explains their exemption of liability. Upon completion and processing of paperwork you will receive a confirmation of your child's enrollment. Thank you.
